Insurance:
We accept all Medicare patients and some PPO insurances (Signa HealthCare). Some insurance companies do cover the doctors’ services as an out of network provider. We require that you check with your insurance carrier(s) regarding our participation in your specific network. It is your responsibility to update us with any new card that you receive from your carrier.
As a courtesy, we will bill your insurance company. You must provide proof of insurance, insurance name, policy numbers, address, place of employment and any other pertinent information. You are responsible for all deductibles, co-pays and charges not covered by your insurance. All co-payments will be collected at the time of service.
For patients who do not fall into one of these categories, we ask that payment be collected prior to the date of service. We accept checks and credit cards.

For your convenience, our patient and physical therapy paperwork is available online. Please download, print, and fill these out prior to your visit. Click the links below to download the documents. These forms will be a diagnostic tool for Dr. Hopkins and must be filled out completely. These forms may not be completed or submitted online.

NOTE: Bring in all Original MRI and/or CT imaging discs.
If you are unable to print these forms, call the office and request that these forms be mailed (allow three to four days for delivery) to you before your appointment. We can also email you these documents or you can come by the office and pick them up before your appointment date. If you are unable fill these out prior to your appointment date, please be prepared and arrive 30 minutes to an hour in advance of your appointed time to fill out the necessary paperwork.
New patients need to bring in their completed paperwork, picture ID, health insurance card, payment for co-pay, and all original imaging including CT scans, x-rays and MRI’s. We accept American Express, VISA, MasterCard and the Discover Card.
