Scheduled Appointments:
After scheduling your appointment with us, please arrive 15 minutes before your scheduled time. This will allow us enough time to process paperwork required for your visit.
New Patients:
- Bring in all original imaging including CT Scans, X-Rays and MRI’s
- Completed patient forms:
- Picture ID, health insurance card, payment for co-pay or co-insurance.
Requirements for Visiting with Us:
- Bring all current insurance card(s) for every visit you schedule with us.
- Obtain and bring any required referrals from your insurance plan(s).
Cancellations:
To cancel an appointment with us, please let us know at least 24 hours ahead of time. This gives us the ability to offer availability to another client that requires our attention.
We do understand that your time is valuable and we always give our best effort to make every appointment on time and follow schedule; however, emergencies do occur that cause delays. We also do apologize for any inconvenience this may cause.
